Ketamine treatment
Ketamine has been studied in selected chronic-pain conditions, but evidence, protocols and risk vary by diagnosis. Consensus guidance for IV ketamine in chronic pain does not support assuming that every pain condition benefits or that ongoing treatment is appropriate. Assessment by a licensed pain clinician is important.

Ketamine administered via intravenous (IV), intramuscular (IM), or sublingual routes is considered an off-label treatment for psychiatric conditions. Because these applications are not FDA-approved for mental health care, health insurance plans typically do not cover them. Patients in Kerhonkson usually pay out-of-pocket for these services. Before beginning therapy, individuals should request a detailed written estimate that includes the cost of initial evaluations, the specific number of induction sessions, and any long-term maintenance appointments. Additionally, the FDA has issued warnings regarding the use of some formulations; patients should review the FDA warnings about compounded ketamine products.
Spravato (esketamine) is an FDA-approved nasal spray for specific adult depression indications, including treatment-resistant depression. Due to its approved status, it may be eligible for insurance coverage. Coverage often depends on prior authorization, which requires documentation of previous unsuccessful antidepressant trials. Patients must verify benefits directly with their insurance providers. When planning a budget for treatment, patients must also consider the indirect costs associated with repeated clinic visits, such as travel expenses or lodging if navigating the rural roads of Ulster County.
Accessing ketamine therapy or Spravato requires a thorough clinical assessment and a commitment to a repeated treatment schedule. Spravato must be administered in a certified healthcare setting, followed by at least two hours of on-site observation by medical professionals. Due to the sedative effects of the medication, patients are strictly prohibited from driving or operating heavy machinery until the next day, after achieving a restful sleep. A responsible adult must be arranged to provide transportation home after every appointment. In rural Ulster County, winter snow, ice, and long distances can significantly affect travel, so reliable winter transportation planning is essential.
While telehealth platforms may facilitate initial psychiatric assessments or follow-up consultations, they do not establish the availability of at-home treatment. Spravato is never administered at home. Some clinicians prescribe compounded ketamine formulations off-label for home use, but these are not FDA-approved for psychiatric care. The FDA warns about serious potential risks when taking these medications without direct, on-site medical monitoring. Anyone experiencing an acute mental health crisis should immediately contact the 988 Suicide & Crisis Lifeline by calling or texting 988 (available 24/7), or call 911 for emergency assistance.
The first step is scheduling an initial psychiatric evaluation with a local provider or a clinic in the surrounding Hudson Valley. This assessment reviews your mental health history, previous medication trials, and overall physical health to determine if ketamine or Spravato is an appropriate medical option for you.
No. Patients receiving Spravato or in-clinic ketamine treatments are strictly prohibited from driving until the next day, following a restful night's sleep. You must arrange for a responsible adult to drive you home from the clinic after every appointment, as the medication causes temporary impairment.
No, Spravato is never administered at home. Because of strict FDA safety regulations, Spravato must be taken in a certified healthcare setting under direct medical supervision. You will be monitored by clinical staff for at least two hours after administration to watch for any adverse reactions.
Intravenous (IV) and intramuscular (IM) ketamine for psychiatric conditions are prescribed off-label and are not FDA-approved for these uses. Consequently, they are rarely covered by health insurance. Patients generally need to pay out-of-pocket and should request a comprehensive written cost estimate before starting their treatment protocol.
Both Spravato and ketamine therapies require a series of closely spaced appointments, especially during the initial induction phase. Ulster County's winter weather, including heavy snow and icy rural roads, can cause travel delays. It is important to plan reliable transportation with your designated driver to maintain your schedule.
Telehealth can be used for initial psychiatric evaluations and ongoing follow-up care. However, Spravato requires mandatory in-person administration and observation. While some providers prescribe off-label compounded ketamine for home use, this practice is not FDA-approved, and the FDA warns against the risks of unmonitored home use.
Costs, coverage & aftercare
IV, IM and compounded ketamine for psychiatric conditions are off-label; many programs are self-pay. Ask about assessment, medication, administration, monitoring, therapy and maintenance charges separately. We do not assume a statewide price.
Spravato is FDA-approved esketamine, not the same as ketamine infusions. Coverage is not guaranteed. Check eligibility, prior authorization, network status, drug and visit charges, deductibles and copays with your insurer and clinic.
Spravato requires a certified healthcare setting and monitoring for at least two hours. Arrange transport home; do not drive until the next day after restful sleep. For other ketamine treatments, follow your clinician's instructions. Follow-up and maintenance depend on response and tolerability.
Ask who manages your medications, how your usual clinician will receive updates, and whom to contact between visits. Neither a shortage designation nor local depression statistics show that ketamine is appropriate or effective for you. Consult a licensed physician.
Use these questions when you call or book a consultation with any Kerhonkson provider.
Ask whether a physician, psychiatrist or anesthesiologist oversees each session and who you can reach afterward.
A good clinic reviews your history, medications, blood pressure and past treatments before recommending ketamine.
Look for continuous vital-sign monitoring and a calm, private recovery space.
IV infusions, Spravato® nasal spray, IM or sublingual options suit different goals and budgets.
Ask how the clinic coordinates with your therapist or offers integration support between sessions.
Request a written estimate for the first series and any maintenance sessions before you start.
